Summary, etc "Process safety incidents keep happening. Disappointingly, some of the same types of incidents are being repeated. The growth of the chemical and petrochemical industries all over the world has created a whole new audience not versed in the historical record. Bhopal was the worst industrial accident in history. Incidents such as Bhopal can give the industry valuable lessons that lead to development, modification or reinforcement of the need for an element of process safety. After a process related incident, information should be developed in a prudent manner with the goal of communicating this information both internally and externally to foster education about the causes that led to the incident and steps that could be taken to prevent future similar incidents. Having a full view of the entire investigation process as well as the tools and methodologies and best practices from around the world for investigation of chemical process related incidents is important for both experienced and novice incident investigators."--
