HINOGUIN, MSN<br/>Year Completed: 2006<br/><br/>STATEMENT OF THE PROBLEM<br/> The study aimed to assess the adaptation modes in post operative care of the cholecystectomy patient admitted at Cebu City Medical Center, Cebu City.<br/><br/> Specifically, this study sought to answer the following questions:<br/>1. What is the clinical profile of the patient in terms of:<br/>1.4 focal stimuli;<br/>1.5 contextual stimuli;<br/>1.6 residual stimuli?<br/>2. What adaptive modes were utilized by the patient in terms of :<br/>2.1 physiological function;<br/>2.2 self-concept function;<br/>2.3 role function;<br/>2.4 Interdependence function?<br/>3. What nursing diagnoses were identified in the post-operative phase?<br/>4. What discharge instructions were given in terms of:<br/>4.1 medication;<br/>4.2 exercise;<br/>4.3 teachings;<br/>4.4 health teachings;<br/>4.5 observable sign and symptoms to report;<br/>4.6 diet;<br/>4.7 spiritual?<br/><br/>Methodology<br/> <br/>Research Design. A case study provides the researcher with opportunities of having an intimate knowledge of a person?s condition, thoughts, feelings, actions (past and present), intentions and environment.<br/><br/> Research Locale. This case study was conducted at a city-owned government, tertiary training hospital, Cebu City Medical Center. Located at N. Bacalso Street, Cebu City, having the Female Surgical Ward as the specific area of the study.<br/><br/> Research Instruments. The researcher conducted a personal interview with the patient and her two sisters and also from a health care provider. A physical examination was performed on the patient. A review of related literature such as books and journals related to the case was also done. The source of information is primarily the patient and her sisters.<br/><br/> Data Gathering Procedure. The intensive practicum commenced with a course orientation conducted by Dr. Marylou B. Ong. A courtesy call to Dr. Myrna R. Go, Chief of Hospital of Cebu City Medical Center and it was channeled to the Chief Nurse, Mrs. Lyrma S. Soledad. The intensive practicum lasted from the period of April 25- May 1, 2006 and totaled 50 hours of patient contact.<br/> The Roy?s Adaptation Model was used to guide the nurse-researcher in rendering nursing care to the patient.<br/><br/>Conclusion <br/><br/> The researcher has concluded that by using RAM as comprehensive assessment tool, the nurse was able to identify nursing interventions that will assist the patient to achieve highest level of adaptation and independence towards the speedy recovery. Whereby, the health needs of the patient were facilitated, to be more efficient and effective as identified by RAM.<br/><br/>Recommendations<br/><br/> Base on the conclusions, the researcher offers following recommendations are considered:<br/>1. The nurse continuously cares for the patient and support to improve his health condition.<br/>2. The utilization of Roy?s Adaptation Model provides holistic and humanistic care approach in attaining the needs of the patient.<br/>3. The Roy Adaptation Model provides a good support system in helping the patient to gain independence.<br/>4. By utilizing the Roy?s Adaptation Model, so as to guide the nurse practitioner in which the four adaptive modes are useful in identifying the needs of the patient towards optimum health. |
